Dental Services

Simple Dentist PLLC provides professional dental services including, but not limited to, preventive care, cosmetic dentistry, restorative treatments, and emergency dental care. All services are provided by licensed dental professionals in accordance with applicable laws and regulations.

Treatment recommendations are based on professional judgment and patient-specific factors. Final treatment decisions are made collaboratively between the patient and their dental care provider.

Appointments and Scheduling

• Appointments may be scheduled online, by phone, or in person.
• We require at least 24 hours notice for appointment cancellations or rescheduling.
• Repeated no-shows or late cancellations may result in a cancellation fee or dismissal from the practice.
• We make every effort to see patients at their scheduled time but delays may occur due to emergency cases.
• Please arrive 15 minutes early for new patient appointments to complete necessary paperwork.

Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete information about your medical and dental history.
• Inform us of any changes to your health status, medications, or allergies.
• Follow post-treatment care instructions provided by your dental care team.
• Maintain good oral hygiene between visits as recommended.

Payment and Insurance

• Payment is due at the time of service unless other arrangements have been made.
• We accept various payment methods including cash, credit cards, and approved financing options.
• We work with most major dental insurance providers. However, you are responsible for understanding your coverage.
• Insurance estimates are not guarantees of payment. You are responsible for any amounts not covered by insurance.
• Co-payments and deductibles are collected at the time of service.
• Accounts more than 90 days past due may be subject to collection proceedings.

Treatment Consent

Before receiving treatment, you will be asked to sign informed consent forms. These forms explain the proposed treatment, alternatives, risks, and benefits. You have the right to ask questions and refuse treatment. For patients under 18, a parent or legal guardian must provide consent.

Limitation of Liability

While we strive to provide the highest quality dental care, we cannot guarantee specific outcomes. Dental treatment carries inherent risks that will be discussed with you before procedures.
